Dealership found one of the camshaft bearings was "in chunks". They're working through the warranty process, and plan on replacing the cylinder head. We've put a call in to ToyotaCare for a manufacturer buyback/replacement, but it doesn't sound promising until we reach the lemon law period...
Dealership confirmed it's a bad fuel injector, and verifying with Toyota. They should have it replaced in the next couple of days.
Edit: It is NOT an injector. Dealership is working with a Toyota rep to troubleshoot. They're looking at the valve springs as the possible issue now. More to...
