Came with it from factory.

Middle piece?

So you have the front splash guard and then the second little one that covers the oil pan. I'm going to check with my buddy that works at Toyota, but from what I see I'm missing it. Possibly my trim level?

Neither were stock. Stock was only the front heavy duty fabric/felt one. When swapping, I opted to buy both to cover back to the front axle.
