I'm going to change the spare tire to the OEM BFG AT Terrain tire. I'm wondering if there is a way to mount the spare tire so that I can get to the valve stem without dropping the spare.

Sure, just run a hose to it.

Turned the spare over so the valve stem is accessible on each of my pickups for the last twenty years. I don't do any rock crawling, but am not always nice to the under side of my rigs.

Shouldn't see why not, other than as mentioned used heavy off road and will always be a very small risk.
I have always flipped all of mine and keep the stem pointing towards front of vehicle to minimize getting struck.
Haven't done the Tacoma yet as she just completed the last of the Toyota services @ 20k miles and will now be under my care.
