Yeah - that is not unusual.
If there is any front axle binding from when it was in 4WD it will clunk as it disengages the transfer case. Sometimes it helps to drive a short distance with the wheels straight before stopping and moving out of 4WD. Being stopped can help too, but not always.

When my truck's engine is warm and I am not accelerating hard it runs just below the middle. It is normal for it to vary some based on the oil and the temperature, so I would not worry about it.
Yeah, a low priority no doubt and developers are not in charge of what gets fixed and when. Looking at some other long running issues with Toyota trucks it actually might never get fixed.

My first thought is that these engines have two fuel injection systems, port and direct. In some situations they use one or the other and sometimes both. So that might be why you hear different sound levels sometimes and not at others.

That is true for my truck too. Maybe Toyota will eventually update Eco so that it is more effective. People use it thinking it will increase mileage when it actually decreases it in many cases.
I have used Sparks Toyota many times and can recommend them. I was able to buy the Canadian flaps from my dealer and they had them in one day and without shipping the price was pretty much a wash.
Mine has been pretty low too, but as I hit 2k miles it has improved. Today a freeway run at speeds between 70-80 mph got me 20.8, which is a new record. That is on winter blended fuel too, so I think it is turning around from the 14-17 mpg I was getting.
